Primary Care

SKHC encompasses a diverse inter-professional Primary Care team with a wide range of services. The Primary Care team works in partnership with the Traditional Program team. This unique relationhip enables the team to offer our clients high quality primary care while encompassing traditional healing practices.

WHO WE SERVE

Shkagamik-Kwe Health Centre (SKHC) is an Aboriginal Health Access Centre (AHAC) dedicated to providing equal access to quality health care to First Nation, Métis and Inuit individuals and their families who live in the City of Greater Sudbury, or who reside on Henvey Inlet First Nation, Magnetawan First Nation or Wahnapitae First Nation.
We are a culturally based wholistic health centre dedicated to balanced and healthy lifestyles. Programs and services support traditional Aboriginal values encompassing the connectedness of emotional, spiritual, physical and mental well-being.

ACCESSING THE WOVEN BLANKET MODEL OF CARE

The Woven Blanket Model of Care at SKHC involves the mutual commitment by all Staff to work towards the shared goal of improvement in the overall health of an SKHC client. The relationship that the SKHC team have with the client is to support the person to take a major degree of responsibility for his or her care. As part of the “Woven Blanket Model of Care”, the partnership is a genuinely equal one with no helper being subservient nor superior.
Shkagamik–Kwe Health Centre offers a full service medical clinic with a team of practitioners that work with the Woven Blanket Model of care to assist clients to help them lead healthy lives.  Our Primary Care team provides primary medical care coupled with traditional approaches to health and wellbeing.
To become a primary care client of the SKHC Clinic you can call our main site at 705-675-1596 to arrange an appointment with our intake coordinator.  The Clinic is accessible to all First Nations, Inuit and Métis residents residing in the City of Greater Sudbury, Wahnapitae, Magnetewan, and Henvey Inlet First Nations.
